Abstract

This review paper provides a comprehensive analysis of the innovative Sampark Swasthya Model, a community-integrated framework for strengthening primary healthcare systems in resource-constrained environments. Developed by Pallathadka and Debroy (2025) in Assam, India, this model represents a significant advancement in operationalizing community participation within existing healthcare infrastructure. Through a systematic examination of its methodological approach, implementation strategies, and documented outcomes, this review positions the Sampark Swasthya Model as an important contribution to implementation science in global health. The model's five key components: Community Ownership Structure, Integrated Service Delivery, Technology-Enabled Healthcare, Multi-Sectoral Linkages, and Knowledge Management System; collectively transform passive service delivery points into dynamic community health hubs. This analysis reveals substantial improvements in service utilization (37% increase), community participation (52% increase), and health outcomes including immunization coverage (28% increase) and maternal health service utilization (45% increase), with a favorable cost-effectiveness ratio of 4.2:1. By addressing longstanding challenges in community participation through structured implementation phases and adaptive management strategies, the model offers valuable insights for researchers, policymakers, and practitioners working to strengthen primary healthcare in similar contexts globally. The Sampark Swasthya Model's demonstrated success in reimagining Subcentres/Arogya Mandirs provides a viable template for health system strengthening efforts worldwide.
